Participation Agreement Welcome! We are excited to have you join us at a conference or large event. Participation is a privilege that can be revoked, so please read this agreement, registration forms, and related documents carefully. By registering, you (and your parent or legal guardian [if needed]) agree to the policies and accept responsibility as outlined below. If you have questions or concerns regarding this agreement, please reach out to your area leaders at the contact information listed on your event page. If you have trouble connecting with them, please send an email to conference-events@ChurchofJesusChrist.org. Participant’s Commitment. As a participant, I agree that I am responsible for my own conduct. I will live by and support the standards of The Church of Jesus Christ of Latter-day Saints (“Church”) and comply with rules and policies. These include the standards and guidelines published on the website or provided in connection with my registration, as well as any instructions given by Church or peer leaders before or during the event. I agree to live the principles outlined on the event page. I will strive for Christlike behavior. By registering for and attending the event, I affirm that my physical, mental, and emotional wellness will allow me to participate safely. If I need assistance or accommodation due to a personal health condition, I will get advance approval from Church and peer leaders. If a health crisis or other emergency arises that requires additional protocols, I agree to follow any instructions communicated by Church or peer leaders. Subject to applicable law, anything I bring to the event may be searched without notice, including in my room or personal belongings. If I have brought something that is prohibited by law or that violates the event or host facility policies, then it may be confiscated. □ I understand and agree that if I fail to keep these rules and policies or otherwise pose a risk to myself or others, my participation in activities may be restricted, or I may be sent home immediately with no refund. These decisions will be made at the sole discretion of leaders. Parent/Guardian Permission (If Needed) and Consent to Medical Treatment. As a parent or legal guardian of a youth participant, I will support my youth in honoring the commitments outlined above. I will complete (by the date and form required) the Permission and Medical Release Form on behalf of my youth and will provide it to leaders. I give permission for him or her to participate in this event and all event activities unless otherwise indicated to leaders in writing. I authorize the leaders supervising the event to administer emergency treatment for any accident or illness and to act in my stead in approving necessary medical care and any related medical transportation. I will be responsible for the cost of treatment if applicable. I understand and acknowledge that leaders, peers, and host campus entities and their agents are not responsible for the outcomes of medical care decisions, and I consent to necessary treatment with that understanding. I also authorize the medical provider selected by the peers and leaders to disclose protected and personal health information to the leaders and to any physician or health-care provider involved in providing medical care. (If I am a participant of a legal age that does not require parental permission, I give my consent on my own behalf.) Participation in Activities. I will support a safe, healthy, and positive experience at the event. I understand that participants may engage in physical activities, such as walking long distances outdoors, outdoor games and athletic competitions, dancing, and transportation by motor vehicle. Participation in such activities is voluntary. I understand that there may be certain risks involved with these activities, and myself or those I am responsible for will attend and participate in activities only if my or their mental, emotional, and physical health safely allows. Privacy. The Church and its agents and affiliates collect and process personal information through this registration and from Church membership records to facilitate participation and to respond to questions or issues that may arise. I understand, unless I have otherwise been informed, that the Church will handle my personal information in accordance with applicable law and the Church’s Privacy Notice which I may access at any time at www.ChurchofJesusChrist.org. As a parent or legal guardian of a youth participant, I consent to the Church, its agents, and affiliates collecting and retaining the youth participant’s personal information, including sensitive information that I provide such as information about the youth participant’s health and medical needs. I authorize the transfer of the youth participant’s personal information, including sensitive information, to Church headquarters in the United States of America or to other countries that may have less stringent data protection laws than the country in which I reside. If I am asked to participate in a research study or survey before, during, or after the event, I understand that participation is entirely voluntary, and I may withdraw at any point. Any contact information provided as part of a research study or survey (for example, email address or mobile phone number) will be used only for the purpose of contacting me about participating in future studies or surveys. I may opt out of receiving such contacts at any time. Consent to Use Image and Any Submissions. I grant permission for the Church (or its affiliates, including staff or third parties authorized by the Church) to film, photograph, and record me, either individually or as part of a group, in connection with this event. I grant permission to the Church and its affiliates to use my voice, name, image, likeness, or performance (collectively and individually, my “Image”) in all formats and manners (commercially and noncommercially) anywhere in the world, provided the uses are related to this or similar events or the Church. Accordingly, to the extent permitted by applicable law, I waive all publicity and privacy rights to inspect, approve, or be compensated for the use of my Image in accordance with this agreement. In connection with the event, I may directly or indirectly provide the Church (or its affiliates) with creative materials (for example, photographs, videos, audio recordings, writings, artwork, or other materials) for use in a slideshow, on the website or the Church’s website, or for any other purpose (collectively and individually, “Submissions”). By providing Submissions, to the extent permitted by applicable law, I grant the Church and its affiliates with a nonexclusive, irrevocable, worldwide, and perpetual license to reproduce, distribute, create derivative works of, perform, display, transmit, and otherwise use such Submissions in all formats and manners, provided the uses are related to the event or the Church. I represent that I have all the rights needed to grant this license. Transportation. If Church or peer leaders arrange transportation for me to and from the conference location, I understand that this agreement and all event policies will apply during that transport. If Church and peer leaders are not arranging transportation for me, I will be responsible for arranging all transportation to and from the event. General. I understand that the Church engages third parties to help administer and promote conferences, and while those third parties are not parties to this agreement, they are intended beneficiaries. This agreement will be governed by the laws of the country or jurisdiction of Texas, except for its conflict of law rules. By providing your mobile number, you consent to receive text messages about this event. Message frequency varies. Msg & data rates may apply. Reply STOP to opt out. Additional Waiver of Liability for Host Venue. Some third-party housing providers and host facilities might require a waiver and release before participants may enter the housing or host facility. To help satisfy these requirements, this additional waiver is included and is a binding part of this participation agreement. I agree to assume all responsibility and liability for any damage or harm caused to person or property due to willful or reckless conduct. I recognize I am a guest at the host facility and that my actions for good or bad will reflect on the Church and the program. I will respect the facilities and personal property of others, will treat them with care, and will be responsible if I intentionally or recklessly cause damage. I will not take legal action against a third-party housing provider or host facility absent willful or reckless action or omission by such entities, but this waiver does not apply to any rights that cannot be waived under applicable law.
